How To Use A Fax Machine

Alright, well, in order for me to claim money from a site where I have bet on some games, I need to send them a fax. However, I have no damn clue how to do it.

 

The biggest problem is that I need to print out the form and then fax it because I need to make an imprint of my credit card. How the hell do I fax that?
